Home Care Packages (HCP)

Quality home care empowering you to thrive.


Remaining in your home for longer is possible with a Home Care Package (HCP). A Home Care Package involves a personalised plan of care services that will support your health and wellbeing in the comfort and familiar surroundings of your own home.

As a respected care provider, the Australian Chinese Community Association of NSW (ACCA) has provided a range of services and support tailored to the Chinese community in Sydney since 1974.

We work for you and with you to produce a personalised support plan, create a budget to guide your care and schedule your treatment and medical needs. Each plan is continuously updated to ensure it meets your current and future requirements, adapting seamlessly to any change in your circumstances.


Our home care services include:

 

Personal care

Assistance with bathing, dressing, personal hygiene, and grooming

Social support

Accompany visits to the doctor, shopping, and social activities

Domestic assistance

House cleaning, laundry, ironing and making beds.

Nutritional needs

Meal preparation, meeting dietary needs, cooking, and cleaning up

Home maintenance

Gardening, repairs, and home modifications related to your care

Extra needs

Dementia care and respite care is available upon request

Medical assistance

Skin integrity, continence care, and medication management

Allied health and nursing

Physio, exercise physiology, occupational therapy and podiatry

Our primary aim is to help you and your loved ones feel safe and comfortable by fostering a supportive and trusting relationship between you and our caring professionals.

What are Home Care Packages?

Each HCP is funded by the Federal Government under the My Aged Care program, to find out more, click here. The government pays ACCA a subsidy to arrange your personalised plan and we consistently meet a strict set of rules and quality standards as regulated by the Aged Care Quality and Safety Commission.

Each HCP fee structure includes the subsidy provided by the Australian Government and the contributions you may be asked to pay, being:

  • A basic daily fee, and/or,
  • An assessed amount based on your ability to contribute called the income tested care fee (ITCF) and/or;
  • Any fees you agree to or can afford to pay.

HCPs can provide four different levels of care and support.

Each level receives a varying amount of government funding. The higher the level, the greater the funding provided by the government on your behalf, enabling you to purchase more services. A level 4 package offers the opportunity to purchase the most services.

HCP funds are not paid directly to you. Once you select an approved HCP provider, such as ACCA, we assist you in purchasing hours of care or other support services to meet your needs. ACCA administers the funds on your behalf and provides case management support and advice, allowing you to focus on living your best life.

ACCA will review your HCP and if your needs change, we will further tailor your plan and this can mean an increase in the level of support required.
