Community services provided by ACCA

ACCA provides a wide range of government-funded home and community care services. The majority of ACCA’s service staff has formal qualifications in nursing, social work, and/or aged care. They are supported by an efficient administration team based at head office in Surry Hills. Services are provided in the homes of clients, at community centres, or on ACCA’s own premises.

Home Care Packages (HCP)

Funded by Department of Health, these services provide high quality affordable and flexible care for the frail older people so that they can continue living independently in the home of their choice as long as possible. These services also support family members, friends and neighbours in their caring role. The type of services include the following – companionship, shopping, light housework, transport to appointments, bill payments, banking, assistance with cooking, showering and dressing assistance, grooming, and outings. A staff is available on 24/7 rotating roster.

Commonwealth Home Support Programme (CHSP)

This programme is funded by Department of Health and is a consolidated programme that provides entry-level home support for older people who need assistance to keep living independently at home and in their community.

Flexible Respite

Respite care is a form of support for people living with their carer. We aim to ensure the carer feel relax and stress-free while taking a break from their demands of their caring role. Whether it's assisting care's loved one with daily task, taking them to appointments, or spending time with them, our friendly home care workers will care for carer's loved one in a kind and considerate way, giving carer peace of mind.

Centre-based Respite Care Centres
The aim of this service is to contribute to the support and maintenance of caring relationships between carers and their dependent family members by facilitating access to information, respite care and other support appropriate to their individual needs and circumstances. Direct respite care will be provided in day care centres. It allows carers to have a short-term break from their usual caring role.

MingShi Dementia Centre-based Respite Care
The objective of the Dementia Day Centres is to relieve pressure on families and carers. The dementia care-recipients from Chinese background are afforded the opportunities to socialise and meet new friends. Help are provided for them to engage in activities that maintain their physical and mental well-being. Support for carers, information and educational programs, and referral services are also provided.

Community and Home Support 

 

Social Support - Group
The service provides group activities to the frail aged Chinese tailored to their requests, needs, abilities and interests. It assists in the development and maintenance of clients’ abilities for independent living, and social interaction. In addition, it also allows the carers and families to have a break.
The program includes social activities, health and welfare talks, gentle exercise, excursions/ visits, handicrafts, games etc. Chinese meals at reasonable price are provided, and transport can be arranged if necessary. A small fee is charged for meals, transport (community bus service) and special programs.

Social Support - Individual
Services provided include provision of community resources, referrals, telephone support service, volunteer home visit for the elderly,  counselling and emotional support.

Meals Service
This service provides “Meals on Wheels” and centre-based meals for the elderly (65 years and above) and people with disabilities. Meals are delivered to their homes by our enthusiastic volunteers from Monday to Friday and are also available at the centre.

ACCA is a registered NDIS service provider aiming to provide support to families, through a series of disability support services, to assist NDIS participants to not only maintain their everyday living, but to achieve greater independence and improve their outcomes later in life. 
 
With experienced and qualified disability service staff, we are here to support people from a Multicultural background with the following service needs:
1. Support Coordination -assist participants in implementing NDIS plan, from coordinating services, sourcing support and translation needs;
2. Assistance with activities of daily living;
3. Assist participants in carrying out household tasks like house cleaning, laundry etc;
4. Community Participation- assistance with participation in community based activities;
5. Assist-Life Stage - Establishing volunteer assistance within the participant's home or community to develop skills;
