Commonwealth Home Support Programme (CHSP)
A little assistance making a big difference
When independence is your main goal and you only need support for a few daily tasks, the Commonwealth Home Support Programme (CHSP) is what you need. The CHSP can assist with entry level support for the elderly, in the short-term or on an ongoing basis, so they can build on their strengths and abilities to function autonomously and remain safely at home.
At the Australian Chinese Community Association of NSW (ACCA) we understand care needs for the aging, from small tasks to high-level care, because we have been providing professional aged care services for 50 years.
We listen to you and tailor our services according to your requirements. When you share your needs with us, our dedicated team will work with you to create a plan that meets your needs and goals, both from a social and health aspect.
Our CHSP services include:
Social support - group
Group activities, gentle exercise, excursions/visits, and specia programs
Centre-based respite / Day Care
Centre-based day respite, community access for groups, dementia support respite and carer specific support for individuals and groups Day Care details
Flexible respite
In-home day respite and other planned respite
Meal services
Meals on wheels and/or centre-based meals
Social support - individual
Accompanied activities, e.g. shopping, telephone/web contact and visiting
Each plan is personalised to your situation and will be comprehensive in its detail on the services or support your unique situation deserves. We offer a continuum of care options because experience has told us that care needs change over time and increased frailties impair your ability to continue living in your own home.
The Commonwealth Home Support Programme (CHSP) is funded by the Australian Government, under the ‘My Aged Care’ system, who then subsidise service providers, such as us at the ACCA, to deliver the services to you for a subsidised price. It's intended for seniors needing entry-level support and simple everyday tasks.
Our professional staff will guide you every step of the way from determining eligibility to providing care regarding CHSP services.
Key eligibility requirements
- If you are over 65 (50 years or older for Aboriginal and Torres Strait Islander people)
- Eligibility of Medicare
- Require entry-level support
If you are eligible, we will help you apply for an assessment, which we will conduct in your home or at a location convenient to you.
Once ‘My Aged Care’ have determined the level of care required, a small contribution of the service provided will be charged. At ACCA, we have the exact costings for all services, from the simple services like meals to more expensive complex services like going out for shopping trips.
